Charitable Donation Requests12 February,2018madnad

I get a lot of requests to donate cookies for charity auctions and sales. Unfortunately, I cannot agree to every request, so in order to help me determine which charities I am able to support, please complete the form below. I have already chosen the charities I will be supporting in 2018, but I am now accepting applications for 2019. 

Please complete the form by 1st November 2018, and I will respond before the end of the year if I can support your particular cause.
